Task: Integrate a compact diff summary (files changed, lines added/removed, file list) into the review/approval flow so reviewers see scope of changes alongside AI review scores before approving or rejecting.

Success Criteria:
Review panel shows changed files list with +/- counts next to each AI reviewer's score. Clicking a filename scrolls to or opens that file's diff. Visible in both review_passed and needs_review states.
